Short-Term Travel Claims
General Instructions:
- Download and complete the required sections of the claim form depending on the type of claim. See details by choosing the type of claim below.
- Submit the claim form using the Online Claim Submission Form below.
- You will receive an email with a support ticket about your claim.
- Prepare all necessary documents, receipts or bills relating to the incident and attach them in the support ticket either via email or by clicking on the View Ticket button
Accident and Sickness
- Download and complete sections A, B, D and E of the Accident & Sickness Claim Form.
- Prepare all medical bills and receipts relating to the incident and attach them in the ticket or email that you receive.
Accidental Death
- Complete all sections of the accidental death claim form.
- Prepare the following and attach in the support ticket:
- All medical bills and receipts relating to the incident
- Police Report, if applicable
- Newspaper clippings regarding the incident, if available
- Death Certificate
- Autopsy Report
- All medical bills and receipts relating to the incident
Accidental Dismemberment
- Complete all sections of the accidental dismemberment claim form.
- Prepare the following and attach in the support ticket:
- Medical Report from doctor
- Medical Records
- Police Report, if applicable
- Medical bills relating to the incident
Personal Effects and Money
- Complete all sections of the personal effects & money claim form.
- Prepare the following and attach in the support ticket:
- Estimates from reputable vendors for all lost or damaged items
- The lost or damaged item's original receipt, if available
- Police Report, witness statement, or airline carrier statement
- Claim tickets for losses and/or items damaged while in the custody of an airline
- Pictures, if the property is damaged, but not lost
Trip Cancellation/Interruptiony
- Complete all sections of the trip cancellation/Interruption claim form.
- Prepare the following and attach in the support ticket:
- Copies of the original and replacement travel documents, such as airline tickets
- Any documentation from the carrier, such as a letter from an airline
- Medical Report if the cancellation is due to an illness, whether for the individual or a family member
- Death Certificate, if the cancellation is due to a death in the family.
