General Instructions:

  1. Download and complete the required sections of the claim form depending on the type of claim. See details by choosing the type of claim below.
  2. Submit the claim form using the Online Claim Submission Form below.
  3. You will receive an email with a support ticket about your claim.
  4. Prepare all necessary documents, receipts or bills relating to the incident and attach them in the support ticket either via email or by clicking on the View Ticket button

Please download the Personal Accident Claim Form and submit together with the following:

  1. Narrative of the incident
  2. Copy of the Confirmation ID and Certificate of Cover
  3. Medical Certificate indicating:
  4. Treatment administered
  5. Final Diagnosis
  6. Receipts
